Bonnie S. is a 101-year-old female who presented to St. Lukes the Vintage for new onset atrial fibrillation possibly due to GI bleeding and anemia. Physical, Occupational, and Speech therapy services created plans of care that were individualized for Bonnie and following 18 days of skilled physical rehabilitation, she demonstrated significant improvements.

Rita D. is a 76-year-old that was diagnosed in the hospital with sepsis. She was admitted to Park Manor CyFair following her stay at Memorial Hermann Cypress. Rita was evaluated by Physical, Occupational, and Speech therapy services.

Pablo S. is a 89-year-old male with history of A-fib, diastolic CHF, CKD, diabetes, hypertension hyperlipidemia, pulmonary fibrosis and sick sinus syndrome. Pablo presents to the ED at St. Lukes the Vintage via EMS from home for evaluation of generalized weakness.

Annie L. is a 97-year-old who initially presented to St. Lukes the vintage with altered mental status following confusion at home. Mrs. Lott was diagnosed with transient ischemic attack. Annie was treated for these conditions with positive results and discharged to Park Manor CyFair.

Mary B. is a 79-year-old who admitted to Park Manor CyFair following a hospital stay at St. Lukes Medical Center. She was diagnosed with Malignant Neoplasm of Pancreas.

Fracture + Cystitis + UTI

Virginia C. is an 80 year old female who had a history of falls in the past and was admitted to HCA North Cypress due to suspected fracture of left sacrum, acute cystitis, and a UTI. She was treated for these conditions with positive results and then discharged to Park Manor CyFair for post-acute skilled Nursing and Rehab services.

 Mrs. Suzanne L. is a 79-year-old that had a recent fall at home. She presented to Methodist Willowbrook and was diagnosed with a sacral fracture. Following her hospital stay she was admitted to Park Manor CyFair.

 Mrs. Nicolasa F. is a 76-year-old female who admitted to Park Manor CyFair following a hospital stay at Memorial Hermann Greater Heights. She was diagnosed with a right femur fracture following a fall. Nicolasa was evaluated by Physical and Occupational therapy services.
